Utilizando el marco analítico de la gobernanza en salud y los tipos de relaciones agente-principal en la provisión de servicios públicos, se realiza un estudio de caso en el que se analiza la configuración institucional en la que se desenvuelve el SNS del Ecuador y cómo ésta afecta la implementación de la política pública. Se observa que, a pesar de los mecanismos diseñados para mejorar la coordinación del sistema, la estructura institucional impide la implementación de las redes integradas de prestación de servicios. O Sistema Nacional de Saúde do Equador (SNS) tem se caracterizado pela sua segmentação e fragmentação, o que condiciona a implementação de políticas públicas no setor e a prestação do serviço. Utilizando o marco analítico da governança em saúde e os tipos de relações agente-principal na prestação de serviços públicos, é realizado um estudo de caso no qual se analisa a configuração institucional em que se desenvolve o SNS do Equador e como esta afeta a implementação da política pública. Observa-se que, apesar dos mecanismos desenhados para melhorar a coordenação do sistema, a estrutura institucional impede a implementação das redes integradas de prestação de serviços.
